How can I obtain a copy of vaccination records from Animal Services?

Customers of Animal Services' Wellness Clinic can request their pet's medical/vaccine records online, via email, by calling 311, or in person at the Wellness Clinic located at the Pet Adoption and Protection Center, 3599 NW 79th Avenue, Doral, FL 33122. The clinic is open Monday to Friday from 9 a.m. to 6 p.m., and Saturday and Sunday from 9 a.m. to 4 p.m.

Please be aware that according to Florida Statute 474.2165 (4), medical records of privately owned pets cannot be provided or discussed with anyone other than the client (pet owner). Our customer service team will require a photo identification to confirm pet ownership.
