Applications must be made in person at the Golden Passport Office. When you apply, your photo will be taken. Your image will appear on your new card.

What to bring:

• A valid Florida Driver’s License or Florida ID showing a Miami-Dade County physical address
• Sample proof of income:
  • Previous Year's Tax Return
  • Previous Year's Tax Return Transcript
  • 1 month of recent paycheck stubs (new applicants only)

The Commuter-Reduced Fare EASY Card is valid for two years and requires biennial renewal.

You can renew by mail, fax, and email or in person at a Transit Service Center. Copies of the required documents must be provided. Your address must be current and correct in our system.

What to provide:

• A current valid Florida ID or driver’s license with a Miami-Dade County physical address.
• Proof of income
  • Previous Year's Tax Return (self-prepared Tax Returns not accepted) 
  • Previous Year's Tax Return Receipt
• Your last issued Commuter Reduced EASY Card.

Replace your lost, stolen or damaged EASY Card at any Transit Service Center by completing the EASY Card replacement form.

Fees:

• $5 for the first replacement
• $20 for the second replacement
• $50 for each replacement thereafter

A fee waiver is available when an original police report is presented, listing the card as a stolen item.
